Since we have PnM FEVER now at GW, I figured I'd post this for reference.

It appears you always get the extra stage if you get at least 125 challenge points (The person who made this page isn't sure though), and clear every stage.  You also get the extra stage at certain specific point intervals lower than that.  The difficulty you can select up to depends on how many challenge points you clear with.  This page also has a listing of each norma and their point value.

That's pretty much how all of the Pop'ns have been since at least 7, but I think the point values vary from game to game.
